不要低估老人跌倒的危险--在马来西亚,每年有超过三分之一的老人跌倒。众所周知,跌倒会导致老人受伤,甚至死亡,因此应该认真对待。

药物和跌倒

为什么老年人首先会跌倒?一个明显的原因是由于年龄大了,身体的协调性和平衡性受到影响。然而,另一个经常被忽视的、可能引起较高跌倒风险的风险因素是药物,特别是一次服用多种药物(称为多药联用)。

我们的老年人口是最大的药物使用者群体,他们服用各种各样的药物--从降压药,到止痛药和治疗痴呆症的药物--你都知道。在这些药物中,有一组药物与跌倒的重大风险有关,被称为 "跌倒风险增加药物"。它们是:

  1. 抗高血压药物:用于治疗高血压的药物。跌倒的风险是由血压的下降引起的。
  2. 利尿剂:也被称为 "水丸",用于排出体内多余的液体。它们通常用于治疗心力衰竭。当液体离开身体时,血压下降,可能会引起头晕和跌倒。
  3. β-受体阻断:它们是一组药物,也是抗高血压药的一部分,同时也用于治疗心力衰竭。患有焦虑症或甲状腺功能亢进症的人确实也会服用β-受体阻滞剂以减少震颤和心悸。同样,β-受体阻滞剂也能降低血压,并能引起头晕。
  4. α受体阻断剂: 常用于治疗前列腺问题。这些药物可引起血压的强烈下降,称为正压性低血压,往往可导致老年人跌倒
  5. 三硝酸甘油酯(GTN): 这种药物是为治疗胸痛而开的,患者在出现胸痛时将一片药片放在舌头下。然而,这种药物也能迅速降低血压,从而增加跌倒的风险。
  6. 镇静剂和催眠剂:用于治疗焦虑症、抑郁症或改善睡眠。这些药物可引起嗜睡,从而增加跌倒的风险。
  7. 神经安定剂和抗精神病药: 用于治疗精神病和其他精神健康障碍。这些药物可引起嗜睡,从而增加跌倒的风险。
  8. 抗抑郁药: 这些药物可引起嗜睡,从而增加跌倒的风险。
  9. 苯二氮卓类药物。也经常用于治疗焦虑症、抑郁症或改善睡眠。这些药物可引起嗜睡,从而增加跌倒的风险。
  10. 麻醉剂: 治疗严重的疼痛。这些药物可引起嗜睡,从而增加跌倒的风险。
  11. 非甾体抗炎药:是一组广泛使用的治疗疼痛和发烧的药物。

 

减少老年人的跌倒风险

虽然有些药物确实会增加老年人的跌倒风险,但跌倒是一个多方面的问题,需要采取多学科的方法。与你信任的医疗服务提供者团队合作,减少你所照顾的老人的跌倒风险:

 

  • 预测跌倒的情况和原因,并找到抵消这些原因的解决方案
  • 严格审查和监测药物摄入量(类型、数量、剂量和摄入时间
  • 为老年人单独设计的运动方案,以促进健康的生活方式而不增加跌倒的风险
  • 仔细(重新)评估对辅助和保护性辅助工具的需求,并促进这些辅助工具的正确使用
